Dice Hospital

Players manage hospitals using worker placement mechanics to treat dice-based patients, where lower dice values represent sicker individuals. Hospital staff and specialists are deployed to heal patients on personal boards, earning reputation points with authorities. The player who accumulates the highest reputation through successful patient treatment wins.

Get Ready

Setup the game with these step-by-step instructions before diving into your turn.

Determine starting player
The player who has most recently visited a hospital is the starting player and receives the First Player token, or choose randomly

Distribute hospital boards
Give each player a Starting Hospital board and 2 score markers in their chosen color

Place starting nurses
Each player takes 3 white nurse meeples and places them in the Nurses' Station area on their player board

Prepare ambulances
Take ambulances equal to one more than the number of players (using lowest numbered), flip to show the side with no dice icons, and place in number order in the center

Set up score board
Place the score/round board next to the play area with each player's score marker next to it and the round marker showing "1"

Prepare Department tiles
Shuffle Department tiles face-down, reveal tiles equal to one less than the number of players (in 2-player, First Player chooses to reveal 1 or 2)

Prepare Specialist cards
Shuffle Specialist cards face-down, reveal cards equal to one less than the number of players (in 2-player, First Player chooses to reveal 1 or 2 total between Departments and Specialists)

Place supply tokens
Place all non-nurse meeples, Blood bag tokens, and Fatality tokens in a general supply

Prepare dice bag
Place dice in the bag based on player count:

Draw starting patients
Starting with First Player and moving clockwise, each player draws 3 random dice from the bag

Set starting patient values
Set the 3 dice to values 3, 4, and 5 (player chooses which die gets which value) and place in untreated spaces (top row) of any ward

Select Hospital Administrator
Shuffle Administrator cards, deal 2 to each player secretly, each player keeps one face-up near their hospital (optional for first game)

Distribute reference cards
Give each player a Reference card

On Your Turn

What you can do during your turn.

Draw ambulance dice
First Player draws 3 dice from the bag for each ambulance and rolls them

Reroll invalid results
Reroll any dice showing 1 or 6 until all show values 2-5

Fill ambulances
Place dice one by one starting with lowest values, filling ambulance 1 first then proceeding to higher numbered ambulances

Resolve tied dice placement
When multiple dice share the same value but different colors, the player to the right of First Player decides placement order

Select ambulances
Starting with First Player clockwise, each player takes one filled ambulance (First Player may not select ambulance 1)

Award lowest ambulance bonus
Player who claimed lowest-numbered ambulance takes a Blood bag token and the First Player token

Return unclaimed dice
Return patient dice from the unclaimed ambulance to the bag

Place new patients
Move patients from your ambulance into empty untreated ward spaces (top row)

Handle overcrowding
If insufficient space for new patients, remove existing patients as fatalities (take Fatality token, return die to bag)

Claim improvements in order
In ambulance number order (lowest first), each player takes either one Department tile or one Specialist card from the display

Attach Department tiles
Place new tiles touching another department; exact location doesn't matter

Gain Specialist meeples
When taking a Specialist card, take the matching colored meeple from supply and place on the card

Optional discard for Blood bag
After all players take improvements, each may return one improvement to bottom of its stack to gain 1 Blood bag token

Activate departments
Place available nurses or specialists one at a time into empty departments to trigger healing effects

Follow activation rules
Each meeple can only be placed once per round; each department can only be activated once per round

Heal patients
When healed, increase patient die value by one step and move from untreated to treated space (bottom row) on first healing

Continue treating treated patients
Treated patients can receive additional healing effects in the same round

Discharge fully healed patients
When a patient reaches value 7 or more, immediately move to discharged area

Trigger specialist abilities
When using a specialist, resolve department effect first, then optionally trigger the specialist's special ability once

Use Blood bag tokens
Spend tokens during activation to heal any patient 1 step OR temporarily change any die's color for the remainder of the phase

Deteriorate untreated patients
Each patient still in untreated spaces (top row) has their value reduced by 1

Record fatalities
If any patient drops to value 0 or below, return die to bag and take a Fatality token for your morgue

Score discharged patients
Count discharged patients and score points: 1=1pt, 2=3pts, 3=5pts, 4=7pts, 5=9pts, 6=11pts, 7=14pts, 8=17pts, 9=21pts, 10=25pts, 11=30pts, 12=35pts

Return discharged dice
Return all scored discharged patients to the bag

Score empty wards bonus
Score 5 additional points if all four wards are completely empty

Reset meeples
Move all nurses back to Nurses' Station and specialists back to their cards

Reset patients
Return all treated patients to untreated ward spaces

Return ambulance
Return your ambulance to the center of the table

Advance round marker
First Player rotates round marker to show next round number

Refresh display
First Player moves unclaimed improvements face-down to bottom of stacks and reveals new tiles/cards as per setup

Winning

How to claim victory.

1
End game trigger
The game ends after completing the 8th round (skip Phase 6 in round 8)

Calculate final score
Adjust score by subtracting 2 points for every Fatality token in your morgue

Add Blood bag bonus
Add 1 point for each remaining Blood bag token
4
Determine winner
The player with the most points wins
5
Break first tie
If tied, the player with fewest patients remaining in their hospital wins
6
Break second tie
If still tied, the player with highest total value of remaining patients (sum all dice pips) wins
7
Share victory
If still tied after all tiebreakers, tied players share the victory
